Proxy Borrower Application

The proxy borrower privilege is a service that facilitates research and coursework by allowing designated individuals to check out library materials on behalf of faculty members, undergraduate or graduate students who are registered with the Disability Resource Center or staff who are registered for accommodation with UF’s Office of ADA Compliance. This privilege extends to all UF Libraries.

  • Instructions

    1. Review Proxy Borrower Policy for activation of the proxy borrower card.
    2. Complete the information for the sponsoring patron/primary account holder
    3. Select option for receiving the proxy borrower card:
      • Sponsoring patron will pick up the card
      • An assigned proxy will pick up the card
  • Sponsor's Information

  • 8 digit UFID number (example: 12345678)
  • (example: email@ufl.edu)
  • Only current UF faculty, students, or staff may serve as Proxy Borrowers, and must present their own, current UFID card prior to borrowing materials on behalf of the Sponsoring Patron.

    The Libraries will confirm accommodation registered with the Disability Resource Center or with UF’s Office of ADA Compliance.
